AFFIDAVIT OF TWO DISINTERESTED PERSONS

KNOW ALL MEN BY THESE PRESENTS:

We, VICENTE AMISTAD , of legal age, single and with residence at Pajo Seaside ,
Lapu-Lapu City Philippines , Philippines and EMMANUEL BERAME, of legal age, single and
with residence at Looc, Lapu-Lapu City, Philippines, after having been sworn to in accordance
with the law, do hereby depose and say;

1. That we personally know JINKY MARIÑAS SANCHEJA, we being friends and long-time
acquaintances;

2. That in her Certificate of Live Birth issued by the Local Civil Registrar, it appeared that JINKY
MARIÑAS SANCHEJA was born on September 10, 1984 in Army Station Hospital, Cebu City
;

3. That her parents’ name were Ismael Panugan Mariñas and Andresa Peras Cadungog as
supported in the said document ;

4. That in her Certificate of Live Birth issued by the Philippine Statistics Authority, her name and
the name of her parents are unclear and unreadable ;

5. That we would like to attest to the fact that the information stated in her Certificate of Live Birth
issued by the Local Civil Registrar and Certificate of Live Birth issued by the Philippine
Statistics Authority refers to one and the same person. ;

6. That WE are executing this affidavit not for any remuneration but to attest to the truth of the
foregoing statements for her requirement for employment and for whatever legal purposes
this may serve her best.

7. Affiant further sayeth naught.
