PETITION

PETITIONER, Allen Joy A. Tactay, unto this Honorable Court, most

respectfully states:

1. That she is over 21 years of age and a Filipino citizen;

2. That she is presently residing at #944, Omfeg, Bontoc Ili, Bontoc,

Mountain Province and residing therein since 1992;

3. That she maintains a regular place of work in Poblacion, Bontoc,

Mountain Province;

4. That she is holding her private law office at 2nd flr. Bagwan
Building, Poblacion, Bontoc, Mountain Province;

5. That she has passed the Bar Examinations and admitted to

Philippines Bar on June 6, 2018. Copy of the Certificate of Bar
Membership is herewith attached as Annex “A”;
6. That she has acquired the necessary clearance from the Regional

Trial Court and the National Bureau of Investigation, hereto

attached as Annex “B” and “B-1” respectively;

7. That she has no pending administrative case as evidenced by

certifications issued by the Office of the Bar Confidant of the
Supreme Court and the Integrated Bar of the Philippines hereto
attached as Annex “C” and Annex “C-1”;

8. That she is a member of the Philippine Bar in good standing with

certification from the Integrated Bar of the Philippines, Mountain
Province Chapter. Copy of said certificate is hereto attached as
Annex “D”;

9. The she has not been charged nor convicted in the first instance of
any crime involving Moral Turpitude;

10. That she desires to be commissioned as Notary Public for and

in the province of Mountain Province and possesses all the
qualifications and none of the disqualifications for said office;

11. In compliance to the requirements of the Honorable Court for

Notarial Commission, attached hereto are the following annexes
and made an integral part of this petition:

a. Three Specimen of her Official Signature as Annex “E”;

b. Picture of Desk, Sketch of Prospective Office and sketch of the
location of the office as Annex “F”, “F-1” and “F-2”
c. 3 pcs. 2x2 size ID picture as Annex “G”;
d. Copy of IBP Membership Payment Receipt as Annex “H”;
e. Copy of PTR Receipt as Annex “I”.
 PRAYER

WHEREFORE, premises considered, it is most respectfully prayed of

this Honorable Court that:
1. The Petition for Commission as Notary Public filed by the
undersigned be acted favorably; and
2. Petitioner be commissioned as a Notary Public in Mountain Province.
