Skill 13 : Catheterization in male patient (SINGLE OPERATOR)

Task: Perform catheterization on a male manikin

Satisfactory completion of task: when all relevant aspects of the tasks, governed by the
following principles, are demonstrated:

● adequate preparation of the patient and the tools for the procedure

● conduct of the procedure adheres strictly to principles of asepsis and standard


precautions

● consideration of patient’s safety and comfort

●adequate documentation of the procedure

●recognition of limitations and possible complications

OBSERVATIONS TO BE MADE BY TRAINER


1. PRELIMINARY
Inform patient/obtain signed consent
Put on mask
2. CHECK EQUIPMENT
Prepare equipment on trolley – Sterile Gloves (X1), Folly’s catheter Size 16 (X1), catheter
bag(X1) , Drapes (X2) , gallipot (X3), kidney dish (X2), cotton wool (X 1 pack), gauze (X 1
pack optional ), 10 ml syringe (X2), sterile water (15 ml) , chlorhexidine in water, local
anaesthetic gel .
(You may not have to do this section if there is an assistant- he/she will do it for you )
1. Wash hands
2. Open First layer of cloth covering catheterisation/ Dressing set.
3. Open outer covering of sterile gloves and empty gloves on to second cloth
layer of catheterization/ Dressing set.

OBSERVE STERILITY
3. Effective Hand washing
4. Put on sterile gloves
5. Open second layer of cloth covering catheter set aseptically
TECHNIQUE
6.  Test balloon of catheter
 Prepare/ check lignocaine gel
 Fill syringe with 8 ml of water
 Drape worksite (over penis and under buttock )
 Place kidney dish containing soaked swabs in between patient’s legs
7. Clean surgical site with saline / chlorhexidine in water (NOT in alcohol)– retract prepuce
and clean meatus and glans of penis .
8. Squeeze local anaesthetic gel into urethra, squeeze of penis to prevent anaesthetic gel
from flowing out. Wait for 5 minutes
TIP ( If local anaesthetic gel is not available ,it may be prepared by mixing 2-3 ml 2%
lignocaine with 5 ml KY gel in the a gallipot . The mixture is then sucked up in a syringe .)

10. Put kidney dish containing urinary catheter in between patient’s legs
11. Insert catheter till urine flows out from catheter or about 20 cm from end of catheter
12. Blow up balloon with the 8ml of water in the syringe
(Urine specimen for microscopy, culture and sensitivity may be taken here, if required )
13. Connect catheter to bag
14. Gently tug catheter to check that it will not fall out spontaneously

CONCLUSION
15. Retract foreskin
16. Clean patient – gel / urine etc
17. Remove drapes
18. Tape catheter to thigh
19. Clear equipment
20. Document procedure

REMOVAL
1 Inform patient
2 Wash hands
3 Wear clean gloves
4 Deflate balloon by syringing out the sterile water
5 Gently pull out the catheter
6 Dispose used items in clinical bag
7 Remove gloves and dispose in clinical bag
8 Wash hands
9 Document
