HOME EDUCATION NOTIFICATION FORM

A parent who elects to provide home education shall supply the following information to the superintendent:

1. School year for which notification is made: 2011 – 2012

3. Name and address of person who will be teaching the child the subjects set forth in #5 if other than parent.

4. Full name and birth date of child to be educated at home: ***Your child’s name and birthdate***

5. Assurance that home education will include the following, except that home education shall not be required to include
any concept, topic or practice that is in conflict with the sincerely held religious beliefs of the parent:

A. Language, reading, spelling and writing


B. Geography, history of the United States and Ohio, and national, state and local government
C. Mathematics
D. Science
E. Health
F. Physical Education
G. Fine arts, including music
H. First aid, safety and fire prevention

6. Brief outline of the intended curriculum for the current year. Such outline is for informational purposes only.

A. Language, reading, spelling and writing:


1) Reading books will be chosen by the children throughout the year.
2) Spelling words will be gathered from reading books.
3) Grammar, punctuation and sentence structure will be learned by reading well written works.
4) Writing will be incorporated into the entire educational process.

B. Geography, History, Government


1) Study of basic world geography
2) Detailed study of history focusing on specific topics chosen by the family throughout the year,
this will include Ohio history, U.S. history and world history.
3) Study of cultures and cultural differences (including comparative religion)
4) Comparative study of different types of government and their development through the ages.

C. Mathematics
1) Number puzzles and games as a means of learning
2) Computer Science at an age appropriate level

D. Science
1) Hands on experiments to teach physical, chemical and electrical science
2) Visits to science centers; COSI, Great Lakes Science Center, etc.
3) Nature study, especially during camping trips and hikes.
4) Biological science, physics, geology as desired by family.

E. Health is part of everyday life and is handled as such


F. Physical Education
1) As an active family we feel our children get plenty of exercise and this category is just a part
of living.
G. Fine arts, including music
1) Visits to various museums and art institutes
2) Exposure to almost every type of music through the family collection of recorded music and
music borrowed form local libraries.
3) Hands on exposure to many art mediums, including clay, drawing, painting, textile arts,
woodworking and basic crafting

H. First aid, safety and fire prevention is part of everyday life and is handled as such

7. List of textbooks, correspondence courses, commercial curricula or other basic teaching materials parent intends to use
for home education.

Our books will be chosen from a wide selection of books as we proceed through the year. In addition to the
large selection of books we own, we also have access to several other families’ collections. We have library cards
from several libraries in the area and are frequent visitors.

We have many educational computer programs in many subjects, including science, math, language and
history.

Access to the Internet provides supplementary material (including worksheets and games) on many subjects,
many interactive sites geared toward my child’s age and abilities as well as Google, Wikipedia, and other
online resources.

We make frequent visits to museums and science centers as well as field trips to various types of factories,
farms and other interesting sites.

But the most important basic teaching material we intend to use is the knowledge in our own heads. As
educating parents there can be no more readily accessible source than the knowledge we have gained over the
years and the ability to pass it on in a one on one environment.

8. Assurance that the child will be provided a minimum of nine hundred hours of home education each school year.

9. Assurance that the home teachers have one of the following qualifications:
A high school diploma
A certificate of high school equivalence
Standardized test scores that demonstrate high school equivalence
Other equivalent credential found appropriate by the superintendent

Lacking the above, the home teacher must work under the direction of a person holding a baccalaureate degree
from a recognized college until the child/children’s test results demonstrate reasonable proficiency or until the
home teacher obtains a high school diploma or certificate of high school equivalence.

10. The parent(s) shall affirm the information supplied with his or her signature prior to providing it to the superintendent.
