AFFIDAVIT OF CONSENT

I, ESTRELLA MACAYAN , of legal age, Filipino, married and resident of Taclobo,

Bacong, Negros Oriental, Philippines, having been duly sworn in accordance with law,
hereby depose and state that:

1. I am one of the surviving co-heirs of a portion land, designated as 2639, covered

by OCT 4777-A, located at Taclobo, Dumaguete City, having inherited the
same from my parent, Nicandro Patrimonio Macayan.

2. I have acquired the said property by virtue of succession -from Nicandro

Patrimonio being one of the children of Natividad Patrimonio, the latter being
one of children of Antonio Patrimonio, and the latter one of the children of
Ciriaco Patrimonio and Anastacia Bohol, the registered owner of the above
mentioned property;

3. I hereby signify my written consent, freely, willingly and voluntarily to allow my

ALEXANDER MACAYAN to have an electric connection on the said property;

4. I will take all responsibilities and obligations arising from the grant of this
consent to ALEXANDER MACAYAN. Hence, any conflict arising between the
owner(s) and/or heir(s) of the property, the City Government of Dumaguete
and NORECO cannot be prejudiced thereof.

5. We are executing this affidavit to attest the truth of the foregoing facts as to our
undertaking to give consent to ALEXANDER MACAYAN to support his
application Electrification Permit.
