after duly being sworn to in accordance with law, hereby depose and state that:

1. That I am the registered owner of a motorcycle, particularly described as

3. That the said motorcycle shall be used to replace the motorcycle used for
the MCH Franchise-07DUM-17-1294, more particularly described as

5. That I am executing this affidavit to attest to the truth of the foregoing and
for whatever legal purpose this may serve.
