JOINT AFFIDAVIT OF LEGITIMATION

We, AAA and BBB, both of legal age, Filipino citizens and presently residing at #40 South
Central Aurora Hill, Baguio City, after having been duly sworn to in accordance with the law,
depose and state that:

1. We are the biological parents of CCC who was born on xxx, 2010 at the xxx
Baguio City;

2. Our child’s Certificate of Live Birth was duly registered with the Office of the
Local Civil Registrar, Baguio City under Registry No. xxx (a photocopy of the
Certificate of Live Birth is hereto attached as Annex “A”);

3. At the time of our child’s conception and birth, we were not yet married although
we were not disqualified by any legal impediment to marry each other;

4. We actually and subsequently entered into a valid contract of marriage on xxx,

2013 at the xxx, Benguet and was solemnized by then Regional Trial Court Judge
xxx;

5. Our Certificate of Marriage was duly registered with the Office of the Local Civil
Registrar, La Trinidad, Benguet under Registry No. xxx (a photocopy of the
Certificate of Marriage is hereto attached as Annex “B”);

6. We are executing this joint affidavit to attest to the truth of all the foregoing facts,
for the purposes of complying with the requirements in applying for the
legitimation by subsequent marriage of our son, CCC and for the proper
recording of such legitimation in his civil registry documents and be entitled to all
the rights of a legitimated child and for whatever legal purpose/s this may serve.
