NEGROS UNIVERSITY for having completed the 120-Hour TESOL
CERTIFICATION COURSE;

b) AWARD ELEMENTS LIST issued by TESOL ASIA for the 120-

Hour Certificate in Teaching English to Speakers of Other
Languages (TESOL);

c) LETTER OF REFERENCE issued by DR. PAUL ROBERTSON as

proof of attendance in the TESOL Asia Certification Course held
at STI West Negros University Bacolod City on May 2018;

d) ATTESTATION by RAMON S. MEDRIANO, JR. (TESOL Asia Sales

Executive and Training Manager) that the 120-Hour TESOL
CERTIFICATE, AWARD ELEMENTS LIST and LETTER OF
REFERENCE issued in my favor are authentic and verified
documents;

2. That the above-mentioned Certificates were issued in my name;

3. This affidavit is for the purpose of attesting to the truth of the foregoing facts,
and for whatever purpose it may serve.
