The following is in reference to the following tools and electronics:

 PN: 050-DG137-0000 1 3/8” Dual Gamma Ray (Geiger) Tool
o PN: 003-137500-GRE 1 3/8” Gamma Ray Board
o PN: 003-137500-HVHV 1 3/8” High Voltage Power Supply Board
o PN: 003-00000-TSB 1 3/8” Tool Switch Board

The head voltage for the 1 3/8” Dual Gamma Ray (Geiger) Tool should never
exceed 150vdc, once the head voltage exceeds 200vdc, the 2.2pf-200v capacitors C2-3
on the HVHV & C11 on the GRE (when configured for +POS Pulse) boards fail. All
supplied data indicates that the tool(s) at some point experienced head voltages above
the maximum acceptable head voltage limit (150vdc).

Whether or not the tools were subject to excessive head voltages, the first and
foremost corrective action(s) must be as follows. All Warrior (SDS) equipment must be
checked to ensure no erroneous voltage surges occur. All 1 3/8” Dual Gamma Ray
Tools must meet minimum factory released operational standards. The Power Control -
Ramp Rate %/Sec within the Warrior software must be changed, not to exceed a
maximum Ramp Rate %/Sec. of 50 (25-50 is an acceptable value range). (All supplied
failure documentation reflects a Ramp Rate %/Sec. of 10000).

NOTE: The Ramp Rate %/Sec. within the Power Control Window is a percentage of
the total wire-line power applied over time. When set above 100% the panel will overshoot
the desired wire-line voltage.

It is essential that all measurements made while troubleshooting the GRE PCB
are made with an oscilloscope. Measurements made using a DVM are not adequate.
Every geo-service center must have at a minimum an oscilloscope, Multimeter, solder
station just to name a few and must be accompanied with a complete and up to date set
of mechanical & electrical documentation.

To determine the exact failure cause for the 1 3/8” Dual Gamma Ray Tool (SN:
110943) is nearly impossible to determine at this point. Is it an electrical component
failure, is the operator at fault for allowing the Wireline voltage to exceed 150vdc, or is it
a failure due to surface equipment failure. Production technician(s) can only provide a
basic support, by providing the PROBE field support representative(s) with current
documentation to correct the existing 1 3/8” Dual Gamma Ray Tool failure. Below find
screen shots of the basic power supply and analog signal measurements found on the
–GRE board.
