AFFIDAVIT-COMPLAINT

I, TANANA, Filipino, of legal age, married and a resident of

Unahan sa Toril, Philippines, after having been sworn to in
accordance with law, hereby depose and say that:

1. I am the owner of Gwapa Store (hereafter referred to

as the store, for brevity), located at Unahan sa Toril,
Philippines, as evidenced by a Mayor’s Permit issued
under my name, copy of which is attached herewith as
Annex “A”;

2. The said store used to be full of dry goods such as

RTWs and plastic wares, copy of the pictures of the
store prior to the complained incident is attached
herewith as Annexes “B” to “B-7”;

3. Sometime in the fourth week of March, 2018, after the

closing ceremony of the Pordoy Elementary School, I
decided to temporarily close the store in order to have
a vacation at Digos City, together with my family
members and storekeeper;

4. When we returned from our vacation on May 28, 2015,

I was shocked when I discovered that my store was
already forcibly opened by destroying the padlocks of
the two (2) doors of the said store;

5. The suspects even replaced the padlocks of the store

and nailed a piece of wood on the wooden doors of the
store, thus preventing us from entering my store, copy
of the pictures of the wooden doors of the store is
attached herewith as Annexes “C” to “C-2”;

6. Thereafter, I immediately reported the matter to

Barangay Captain Bahug Duga of Barangay Pindan
and sought the assistance of the police authorities of
Dakugtiyan Police Station to check the premises of the
store and PO1 Mabaho was sent to assist me, copy of
the Police Incident Report Form/Blotter is hereby
attached as Annex “D”;

7. Upon accounting for the inventories of the store, I

discovered that the perpetrators disarranged and took
some of the displayed items consisting of assorted
plastic wares, shelf glass, dura box, among others,
valued at Thirty Thousand Five Hundred (P30,500.00)
Pesos, more or less, copy of the pictures of the inside
premises of the store after the complained incident is
attached herewith as Annexes “E” to “E-3”;
 8. The perpetrators also took some of my personal
things kept in the store consisting of my personal
jewelries amounting to Fifteen Thousand (P15,000.00)
Pesos, more or less, by forcibly opening and
destroying the drawer where my jewelry box was kept,
copy of the pictures of the said drawer forcibly opened
is attached herewith as Annexes “F” to “F-3”;

9. The perpetrators also took my piggy bank containing

assorted coins amounting to Four Thousand Five
Hundred (P4,500.00) Pesos, more or less;

10. The perpetrators also forcibly opened my room where I

sleep located at the second floor of the store, copy of
the picture of the door of my said room which was
forcibly opened is attached herewith as Annex “G”;

11. The total amount of items, including my personal

things, which were taken from my store is estimated to
be at Fifty Thousand (P50,000.00) Pesos;

12. Later on, I learned that the perpetrator is

______________;

13. That I execute this affidavit to charge ________, of

Banate, North Cotabato Province for:

a. Robbery under Article 299 of the Revised Penal

Code of the Philippines; and
b. Qualified Trespass to Dwelling under Article 180
of the Revised Penal Code.
