AFFIDAVIT

I, LORNA C. GUZMAN, Filipino, of legal age, single, and a resident of Centro Enrile,
Cagayan, after being duly sworn to in accordance with law, hereby depose and state
that:

1. I am a permanent employee of the Office of the Sangguniang

Panlalawigan, Province of Cagayan;

2. Sometime on November 20, 2015, I have a personal knowledge that Helen

Salvado borrowed from Dexter Bautista money amounting to P100, 000.00;

3. I am aware of the fact that Ms. Salvado has surrendered her Landbank
ATM Card to Mr. Bautista as a consideration of her indebtedness where
the monthly amortization will be sourced out;

4. I am knowledgeable also that Ms. Salvado’s monthly salary is being

withdrawn by Mr. Baustista for the payment thereof; and

5. As such, I am executing this Affidavit to attest to the truth of the foregoing

and for whatever legal purposes it may serve.

IN WITNESS WHEREOF, I have hereunto affixed my signature this 12th day of
