AFFIDAVIT OF CONSENT

I, DANIEL L. MANLANGIT, of legal age and residing at Purok 26, Riverside, Ma-a,
Davao City, Philippines, after having been sworn in accordance with law, depose and say:

1. That I am the owner of a parcel of land situated at Ma-a, Davao City, covered by
TCT No. T-166908 by virtue of a Deed of Donation executed by my father, Delfin
A. Manlangit;

2. That my brother, RAMIR L. MANLANGIT and his family is currently occupying the
above-mentioned parcel of land;

3. That I as the property owner consents to Mr. Ramir L. Manlangit’s application for
a service connection with the Davao Light and Power Co.;

4. That I am executing this Affidavit of Consent to attest to the truth of the foregoing.

IN WITNESS WHEREOF, I have hereunto set my hand this 24 th day of April 2018, in
