Affidavit of Complaint

I, Bemla Remoto, of legal age Filipino, with office address at #888 Delbros
Avenue, Pascor Drive, Queensway Subdivision, Brgy. Sto Nino, Paranaque City.
after having sworn in accordance with law hereby depose and state that:

1. I am designated as Credit and Collection Assistant of Delbros Waterfront

Chassis Leasing Corporation (DELWATER), a corporation duly organized
and existing in accordance with the laws of the Republic of the Philippines;
2. On March 1, 2011, a contract of lease was entered into by herein plaintiff
and defendant corporation United Logistics Represented by Jose A. Yu with
principal business address Cargo Center 2, CNT Arcade 333, New
Panaderos., Sta. Ana, Manila;
3. On October 15, 2015, DELWATER issued a Statement of Account
amounting to Php 212, 770. 53 and it was received by herein defendant
through Mr. Glendell Tanag,;
4. On June 7, 2017, DELWATER issued another Statement of Account
amounting to Php 212, 770. 53 and it was received by herein defendant
through Mr. Vincent Alviz;
5. On October 11, 2017, DELWATER issued another Statement of Account
through Registered Mail towards the principal address of herein defendant
but they already moved out;
6. DELWATER tried to contact herein defendant through phone call about the
existence of their legal obligation and gave them a discount amounting to
Php 198, 682 but they still refused to settle such obligation;
7. DELWATER sent a demand letter amounting to Php 198, 682 through
registered mail towards the principal address of herein defendant but it was
refused to receive;
8. I am executing this Affidavit to attest the truth of the foregoing and for all
legal intents and purposes it may serve.
