Instructions:
1. Provide facility name, authorized TA name, square footage, and application date.
2. Fill out, Area of Building column, and all non-gray fields under 'Baseline or Existing Lighting' and 'Proposed Lighting'.
3. For 'Area Type’ and ‘Product Qualification’ fields, please make a selection from the dropdown list.
4. Enter only one existing/proposed fixture combination per row, use mulitple rows for areas with more than one existing product combination.
5. Ensure that the model number corresponds to cut sheets included with the application.
6. Once you have completed all fields, save as an Excel (.xls) file and submit with your Custom Rebate Application.
7. If you have questions, please contact your designated Outreach Specialist.
