AFFIDAVIT OF DICREPANCY

That I, DINO L. MARIANO, of legal age, Filipino citizen, and a resident of and with
postal address at Pulis Kalinga, San Vicente, Tarlac City, Philippines, do hereby depose
and say:

1. That I am the father of DINO HERALD NECOSIA and DINARD NECOSIA who
was born on September 12, 1995 and April 10, 1997 a copy of their Certificate of
Live Birth is hereto attached;
2. That in their Certificate of Live Birth their surname was erroneously entered as
‘‘NICOSIA” instead of ‘‘NECOSIA”;
3. That in the Certificate of Live birth of their mother maiden name is registered as
ANNALISA D. NICOSIA instead of PRIMA D. NECOSIA which is true and correct
one;
4. That I am married to PRIMA D. NECOSIA, and our union begot three (3) children,
a copy of our Marriage Certificate is hereto attached;
5. That in our Marriage Contract, my wife’s surname appearing thereon is ‘‘FRIMA
D. NICOSIA” instead of ‘‘PRIMA D. NECOSIA” which is true and correct one;
6. That ‘‘FRIMA D. NICOSIA” and ‘‘PRIMA D. NECOSIA” refer to one and the same
person;
7. That ‘‘DINO HERALD NICOSIA” and ‘‘DINO HERALD NECOSIA” refer to one
and the same person;
8. That ‘‘DINARD NICOSIA” and ‘‘DINARD NECOSIA” refer to one and the same
person
9. That I am executing this affidavit to attest to the truth the foregoing statement and
for whatever intent and purpose it may serve.
