Page 91 – Table 3-17 – Delete "rhinovirus" from Empiric Therapy column for 3 months
to 5 years.

Page 93 – Figure 3-21 – Caption should read, “Normal chest film.”

Page 108 – Table 4-1 – Vital sign values are not correctly placed within the table. Heart
rate values for ages 2, 4, 6, 8, 12, and 14 years are placed under respiratory rate.
Respiratory rate values for 2, 4, 6, 8, 12, and 14 years are placed under blood pressure;
and the associated blood pressure values are to the right. The table should read:

Page 237 – Table 8-1 – In the Syndrome column for cyclic antidepressants, the “quick
release system greater than 100 seconds should say “QRS greater than 100 milliseconds.”

Page 247 – Under Diagnostic Studies in the right column, replace the second sentence of
the first paragraph (A quick release system (QRS) interval duration of greater than 100
milliseconds is a risk factor for dysrhythmia.) with “A QRS duration of greater than 100
milliseconds is a risk factor for dysrhythmia.”

Page 413 – Figure 12-1 – Figure 12.1B correctly shows Shenton line displaced laterally
in a hip with DDH. Figure 12.1A shows an abnormal gas pattern in the abdomen.

Page 428 – Figure 12-10 – Replace this figure and caption with Figure 12-11.

Page 431 – Figure 12-11 – Replace this figure with Figure 12-12. The caption should
read: "Frog-leg view of a left slipped capital femoral epiphysis."

Page 431 – Figure 12-12 – Replace this figure with Figure 12-10. The caption should
read: "Anteroposterior radiograph of slipped capital femoral epiphysis revealing normal
Klein's line on right hip, intersecting the edge of the epiphysis, and an abnormal Klein's
line on the left hip, missing the epiphysis."

Page 435 – Figure 12-13 – Incorrect image and caption. (Malrotation)

Page 507 – Table 15-5 – In the Dose column for Etomidate, change dose to .1-.3 mg/kg.
In the Max Unit Dose column for Etomidate, change dose to 20 mg. In the Comments
column for Pentobarbital, Methohexital, and Thiopental, correct spelling to “porphyria.”

Page 512 – Under Other Sedative/Hypnotic Agents in the left column, delete the second
sentence of the first paragraph, “Used to provide both analgesia and sedation, etomidate
is used for brief painful procedures.”

Page 551 – Figure 17.3 – Change colors to show “Deceased” as black and “Immediate”
as red.

Page 600 – Figure 19.6 – Change label A to read, “Positioning of gastrostomy tube or
PEG in abdomen.”

Page 774 – Chapter 11, Question 3 answer should be “D”. Midgut volvulus.

Page 778 – Chapter 19, Question 1 change the spelling of, “E, Suction tracheostomy.”
