AFFIDAVIT OF PARENTAL ADVICE

I, _________________, 29 years old, single, Filipino, and a resident of Andagao,

Kalibo, Aklan, after having been duly sworn to in accordance with law, do hereby depose
and say:

1. That I am the guardian/older brother of _____________________, 23 years old,

single, Filipino, and a resident of Andagao, Kalibo, Aklan, who was born on October
13, 1994 at Talimagao, Madalag, Aklan;

2. That my said ward/younger brother is intending to enter into marriage with

__________________, 26 years old, single, presently residing at Andagao, Kalibo,
Aklan;

3. That parental advice is being sought by my said ward/younger brother to marry

__________________, and for this purpose, I as his guardian in the absence of our
parents who are currently abroad is hereby giving my advice favorable and have no
objection to his marriage;

4. That parental advice has been given to him favorably for the reason that he is
still single, has all the qualifications to marry and with no legal impediment.

5. That I am executing this affidavit to attest to the truth of the foregoing facts for all
legal intents and purposes.
