Elnas, Jr., Election and Barangay Affairs Department, requesting amendment of the approved revised template of registration form pursuant to Minute Resolution No, 18-0066 dated January 25, 2018, with the recommending approval of Deputy Executive Director for Operations Bartolome J. Sinocruz, Jr. The Memorandum of Director Elnas reads: “This pertains to the newly approved revised template of registration form pursuant to Minute Resolution No. 18-0066 dated vanuary 26, 2018. In as much as the status of voter, whether he/she is a Senior Citizen or not, can be determined or the statistical data can be generated based on the date of birth (age) reflected in the application forms and encoded in the Voter's Registration System (VRS), this Department hereby recommends the deletion of the text box ‘Senior Citizen’ in the upper right portion of Part 1 (Personat information) in the approved new template of application form.
