1.0 Pre-Dive Phase

An Job Hazard Analysis for the diving operation follows:

Phase of Work: Pre-dive
POTENTIAL HAZARDS RECOMMENDED CONTROLS AND
PREVENTION
1. Equipment Failure Equipment should be inspected according to the pre-dive
checklist. The breathing gas supply system including reserve
breathing gas systems, masks, helmets, and thermal protection
(when appropriate) shall be inspected prior to each dive.

2. Diver Injury *Prior to making individual dive team member assignments,


employer should inquire into the dive team member’s current
state of physical fitness, and indicate to the dive team member
the procedure for reporting physical problems or adverse
physiological effects during and after the dive.
*Dive station shall have a cell phone for emergency contact.
3. Unprepared dive team * Diver shall be familiar will all aspects of the dive plan and
member manual, especially the emergency management procedure.
* Divers will be briefed daily prior to diving activity.
*Safety procedure per the safe practices manual.
*modifications to standard operating procedures necessitated
by the specific diving operation.
4. Environmental conditions *An environmental check of surface and subsurface conditions,
and the immediate surroundings should be conducted prior to
the dive.
5. Diver communications *Test voice communications systems prior to dive
malfunction *In addition to standard signals, divers should be aware of any
special line-pull signals to meet specific mission requirements
prior to dive
*Dive operations will be suspended if communications
equipment is inoperable.
6. Water entry and exit *Safe means should be provided to support the diver in
accident/injury entering and exiting the water.
*The means provided for exiting the water shall extend below
the water surface.
*A means shall be provided to assist an injured diver from the
water.
*The propeller of a dive boat shall be stopped before the diver
enters or exits the water.
*Means to minimize the entanglement of the air supply hose
and the vessel propeller should be devised.
7. Contaminated water or air *If sewage or other hazardous pollutants visibly contaminate
the water, the Diving Supervisor shall suspend operations until
conditions improve.
*If diver experiences physical irritation/injury from
contaminated water, the dive shall be aborted and operations
suspended until conditions improve.
*The Diving Supervisor shall suspend operations if polluted air
poses a risk to any dive team member.
8. Adverse environmental *The Diving supervisor will suspend all dive operations if

2.0 During-Dive Phase

PREVENTION
1. Leaks in air supply system *After entering water, the diver should immediately check for
leaks in the air system.
*In case of a leak, the diver will exit the water and the dive will
be aborted and operations will be suspended until the
equipment is safe for use.
2. Problems in descent *Descent to the bottom may be achieved with the aid of a
descent line or stage.
*Topside personnel must ensure that air is being supplied at
sufficient quality and pressure.
*A diver experiencing problems descending should inform
topside personnel and the Diving Supervisor may choose to
abort the dive and replace the diver.
3. Diver requires assistance *A standby diver shall be available while a diver is in the water.
4. Problems underwater *Upon reaching bottom diver should check airflow.
including air supply, currents, *Diver should take time to become oriented to bottom of work
and diver orientation site and note natural features such as current speed and
direction.
*If diver has trouble with orientation, tender can assist diver by
use of line-pull signal.
*If the diver experiences problems with the air flow, the diver
shall communicate with topside personnel and abort dive.
5. Entanglement at bottom of *Diver and tender should make sure umbilical line is not fouled
worksite before descending.
*Diver should loop one turn of lifeline and air hose over an arm
to act as buffer against sudden surge of pull on lines.
*Diver shall request assistance if the entanglement is too
difficult to clear.
6. Strong currents at bottom *If a current is strong the diver can stop or crawl to reduce
of worksite body area exposed to the current. If the current is too strong to
perform the work safely or effectively, the Diving Supervisor
may elect to abort the dive and suspend the operations until
conditions improve.
7. Damage to lifeline *Diver should ensure that lines do not become fouled on
outcroppings or other sharp projections. The tender must also
be careful to take up any slack to avoid fouling. If the lifeline is
damaged, the dive shall be aborted and operations suspended
until diver safety is not compromised.
8. Fouled lifeline *If line is fouled diver should stop and examine situation. Diver
will have backup air supply in case of air-flow interruption.
Diving Supervisor should be notified and procedures is followed
for fouled lines. Diving Supervisor may choose to abort dive.

9. Sharp object penetration *Divers shall wear protective clothing (wet-suit and/or
coveralls).
*Divers shall be briefed on potential dangers during daily pre-
dive meeting.
*Dive team shall be familiar with first aid for potential injuries.
10. Air Failure *Diver will turn on emergency air supply.
*Diver will notify personnel topside.
*Tender will turn on backup air supply.
*Dive will be aborted.
11. Mechanical hazards *Topside personnel will secure objects that may accidentally
fall.
12. Decompression sickness *Dive supervisor shall ensure no decompression diving. Dive
plan must address depth and time of dives. If decompression
diving becomes necessary, a new dive plan will be submitted to
USACE for approval.
13. Diver injury *Diver will not perform work that is not encompassed in the
dive plan.
*Dive team will be briefed on response to injury procedures.
14. Problem occurs while *There will be a Diving Supervisor on site at all times.

15. Drowning *Immediate assistance by standby diver.

stamina and dive conditions.
*Take rest stops, if necessary, to prevent overexertion.
*Maintain and use proper equipment at all times.
18. Intake air is fouled by *Diver tender will make sure that the intake of the compressor
generator is upwind of and away from any exhaust fumes, including that
resulting from the generator of traffic.
19. Underwater Welding *Diver tender is to make sure airlines are away from any hot
work.
*A hot work permit is to be obtained prior to any welding
activities.
*The diver and tender are to ensure proper set up of the
welding system, to include grounding.
*The diver welder is to keep the stringer clear from his diving
equipment at all times.
*When not actively welding, the rod must be removed from the
stringer.
*All equipment must be taken to avoid electric shot injuries.
*The diver welder must personally ensure equipment is set up
properly prior to each used and stored properly at the end of
the day.
*The diver welder must ensure the area is acceptable for
welding and all safety precautions have been taken prior to
each operation.

3.0 Post-Dive Phase

PREVENTION
1. Flying after diving *Decompression diving will not take place. Divers shall not fly
for twelve hours following a no-decompression dive. This
interval should be extended to 24 following multiple days of
diving.
2. Injury *Diver will perform self-examination to ensure that no injuries
were suffered during dive.
*Equipment shall be inspected and maintained properly for
safety of future dives.

Basic Life Support

Assess scene safety. Remove the diver from the water if necessary, and ensure
the safety of all rescuers.
1. Assess responsiveness. If the diver is unresponsive, immediately activate
emergency plan. Call for emergency medical services.
2. Open the airway and check for breathing.
3. If the diver is not breathing normally, deliver 2 rescue breaths.
4. Begin CPR. Immediately deliver 30 compressions followed by 2 breaths.
Provide emergency oxygen, if available, at the highest possible
concentration using the most suitable delivery device.

If Emergency Medical system (EMS) help is not available, immediately transport


the injured diver to the nearest emergency medical facility. Provide emergency
department personnel with details concerning the accident, included below.
