AFFIDAVIT OF TWO DISINTERESTED PERSONS

WE, AIDA RUPA MARTINEZ and MARIA HAIDE R. PATANO, both of

legal age, Filipinos and with residence address at No. 9-C Duhat St., Old
Cabuyao, Sauyo, Quezon City and 11-B Old Cabuyao, Sampaloc St.,
Sauyo, Quezon City, respectively, after having sworn to in accordance
with law, do hereby depose and state:

1. That we personally know HERMINIGILDA R. ABIAN, born on

November 27, 1934;

2. That we knew for a fact that during the younger years of

HERMINIGILDA R. ABIAN, her mother made him bear her other name
“AIDA R. ABIAN” making her acquaintances often call her the latter
although his true name is HERMINIGILDA R. ABIAN;

3. That the names HERMINIGILDA R. ABIAN and “AIDA R. ABIAN”,

refers to one and the same person;

4. That we have executed this Affidavit to attest to the truth and

veracity of all the foregoing and for whatever legal intent and legitimate
purpose it may serve and for the information of all authorities/persons
concerned.
