I, EDDIE BOY LAYO., Filipino, of legal age, single, and a resident of Cordillera
St. RESA 1, Puyok, Meycauayan, Bulacan after having been duly sworn in accordance
with law, hereby depose and say:

1. That I am currently processing my application to enroll at the UST;

2. That currently, I have not processed any other application to any technical/
diploma/degree program;

3. That I have no intent to submit any other application in another institution or

college to take any technical/ diploma/degree program;

4. That I am executing this affidavit to attest to the truth and veracity of the
foregoing facts and for all intents and legal purposes.
