1. It is certified that I am eligible to buy a Scooty (Honda Activa) through CSD as per
existing instruction on the subject.
2. It is certified that I have not purchased any two wheeler vehicle from CSD as per
eligibility given below in the form.
3. I am eligible to buy a two wheeler through CSD and not utilizing the facilities of CPO
Canteen Services.
4. It is also certified that vehicle applied is for my personal use and will not be
sold/transferred for four years.
5. It is also certified that I have got the entry made in pay book as per the rules (certified
copy attached).
6. I affirm that the above details submitted are correct and conditions shall be adhere to.
7. I am liable to pay the Excise/Sales Tax (applicable for Civil Defence Personnel)
8. I am liable to pay the entire Taxes exemption availed by me in case of any violation
of above conditions of purchase.
