AFFIDAVIT of EXPLANATION

I, MARY JOY M. BRIMON, of legal age, residing at Purok 1, Brgy. 48

– Bagong Abre, Legazpi City, after having been duly sworn to in accordance with
law, hereby depose and say:

1. That I recently applied for passport with the Department of Foreign

Affairs;

2. That in the application form which was made online and accomplished by
Juvy Almayda, employee of the agency which helps me process my
application to work abroad, it was indicated therein that my civil status is
SINGLE when in truth and in fact I am already MARRIED;

3. That I was married to Jemar Clapis on March 2, 2015 in Sangay,

Camarines Sur;

4. That said mistake was inadvertently made and there was no intent to
mislead or deceive;

5. That I have executed this affidavit to explain the mistake in accomplishing

the application form of the DFA.

IN WITNESS WHEREOF, I have hereunto affixed my signature this
