Subject: Incident Report for Incident of Flood at Basement of Utar KB Block Building on
15th February 2016

This incident report is prepared as part of the next Utar Senate meeting and as part of
Universal Underwriter Inc. , IBP’s insurance carrier and the Department of General
Services. A flood happened in the basement of Utar KB Block on the 15th February 2016
at 1.30a.m. at night after a heavy rain. The basement of the KB Block consists of parking
lots as well as workshops belonging to the Engineering department.
Detailed Description of Incident
The flood happened on 15th February 2016 at 1.30 a.m. in the basement of the Utar KB
Block building. There was multiple witnesses: mostly security guards of Utar KB Block
building. The head of security department, Mr. James responded to the incident and
called emergency services at 1.45a.m. and led all security guards to a safe place. The
entry of outsiders to Utar KB Block were prohibited. UTAR immediately requested the
head of the engineering team, Alice to investigate the flood and make repairs needed to
stem the water flow. There was no any casualties caused by the flood. However, there
were some serious damage to the property located in the basement during the flood. Five
Utar vehicles were flooded which the models of this five vehicles are Toyota Avanza and
Toyota Hillux respectively. In addition, the equipment in mechanical lab was damaged
and furniture stored in the basement was damaged beyond repair.

Causality for Incident


After investigation, as the head of engineering team, I have established the following
causes for the flood in the basement of KB block:
1. The outlets in basement drainage system have been blocked. Thus, the rainwater
in the basement area could be drained properly to remove it and caused the
happening of flood.
2. According to weather forecast, monsoon drains has been lasted for one month in
Bandar Sungai Long. Thus, it has resulted in the heavy rain for continuously few
days.
Proposed Remedial Actions
1. Short term: The safety department has called in the contractor to clean the
blocked outlets.

2. Medium term: The safety department recommends that university authority


should hire some engineers to design a flood evacuation system and process in the
campus to prevent flood in the future.

3. Long term: The safety department recommends that additional flood mitigation
system such as mini small tunnel to be built in the campus.
