RE: REQUEST TO HOLD MERALCO ELECTRIC SERVICE/LINE

SIR:

Greetings!

I would like to request your good office to hold the meralco electric service/line
installation for ______________ due to the following:

1. CONFLICT WITH THE PAYMENT OF THE CASH BOND

____________________ is a beneficiary of the Housing INc. with unit specifically
located at _________________. In relation to our application with Meralco, her cash bond
has been paid in advance by the Phase 3 subject to subsequent reimbursement as agreed
by the beneficiaries in a meeting set for the purpose. Recently we have learned that she
gave corresponding payment to the President of Phase I which is not in consonance with
the internally agreed procedure. With this recent event, her account is subject for review
and resolution;

2. REQUIREMENT OF ELECTRIC SERVICE/LINE INSTALLATION

Based on the previous letter of consent made by Ms. XXXX , President of ARCHANGEL
SWORD INC., consent for MERALCO to proceed with their Electric Service/Line Installation was
limited to individual applicant-beneficiary of Housing Inc. whose residential lot payment is updated
until December 2017. At this moment account of +______________________ is subject for review and
approval as we have not received any payment hence, her account is not updated as required;

3. AUTHORITY OF HOUSING INC., THROUGH ITS OFFICERS AND MERALCO

TO REVOKE / SUSPEND ELECTRIC SERVICE/LINE INSTALLATION
Based on the same letter of consent , Housing INC., through its officers and MERALCO reserve its
authority to revoke any Electric Service/Line Installation application or refuse to complete or continue
with Electric Service/Line installation by reason of deficiency or delinquency in its residential lot
payment until December 2017 and other payments (for materials, installation and processing) as well as
any other documentary requirements as required from the beneficiary.
