accordance with law, hereby depose and say:

1. That I am self-employed as I have no employer other than myself and derive my income
from my physical and mental efforts as accounting consultant in the area of legal aspect
and government compliance from April 30, 2018 to present at Arellano Maldo and
Associates CPA’s Co. where I generate income in the amount of Php. 28,000.00 for
service rendered to two clients referred as of this date;

2. That I am executing this affidavit to attest to the truth of the foregoing instrument and
for any legal purposes and intent this may serve.
