CONTRACT of services

KNOW ALL MEN BY THESE PRESENTS:

This Agreement, made and entered into this _30th day of August, 2016 by and
between:

The PROVINCIAL GOVERNMENT OF QUIRINO, a local government unit with

business address at the Capitol Hills, Cabarroguis, Quirino, represented by
the Honorable JUNIE E. CUA in his capacity as Governor, hereinafter referred
as the FIRST PARTY;

And

ARIVA EVENTS MANAGEMENT, INC., a private business address at Unit 305

Metropolitan Avenue corner Sacred Heart & Kamagong Streets, San Antonio
Village, Makati City represented by MS. REGINE ANINGAT, Finance and
Administrative Officer, hereinafter to as the SECOND PARTY;

W I T N E S S E T H:

WHEREAS, the FIRST PARTY will conduct its 45th QUIRINO DAY dubbed as
“PANAGDADAPUN FESTIVAL” on September 5-10, 2016 at the Provincial Capitol Compound,
Capitol Hills, Cabarroguis, Quirino Province.

WHEREAS, the FIRST PARTY has signified its intentions to hire the services of an
experienced event coordinator to provide full Band Set-up, LED Wall and live feed video
coverage on particular days during the occasion, as follows:

a. SOUND SYSTEM - September 5,6,7,9 & 10;

b. 3 Units LED WALL ( 9ft x 12 ft) - September 8;
With live feed video coverage
c. 1 unit LED WALL (9ftx 12 ft) (Oval) - September 10, 2016
With Live feed video coverage
d. SOUNDS AND LIGHTS (morning to Evening) - September 8.

For a total package amount of FOUR HUNDRED EIGHTY FIVE THOUSAND PESOS (PhP
485,000.00) inclusive of tax;

WHEREAS, the SECOND PARTY has offered it service for the amount of so stipulated
and undertakes to provide the sets contained in its proposals copies of which are hereto
attached and made part hereof;

WHEREAS, the FIRST PARTY has ascertained from official records that the SECOND
PARTY possess the capability and integrity to execute the services needed;

WHEREAS, as the nature of the service is highly technical and primarily confidential
where trust and confidence are underlying considerations in the hiring, the FIRST PARTY opts
to exercise the provision of Section 53 (f) of the implementing rules and regulations of
Republic Act No. 9184, hence this negotiated procurement.

WHEREFORE, premises considered, the PARTIES agree and covenant as follows:

 I. REPRESENTATION OF THE SECOND PARTY:
1. To provide to the FIRST PARTY the services and the equipment as outlined in the
proposals together with the desired outputs such quality and specification
mentioned in this agreement;

2. To hold the FIRST PARTY free from any liability arising from loss, damage or injury,
including the death while in the performance of the agreement;

II. REPRESENTATION OF THE FIRST PARTY

In consideration of the faithful performance of the representations made by the SECOND

PARTY, the FIRST PARTY shall pay the amount of FOUR HUNDRED EIGHTY FIVE THOUSAND
PESOS (PhP 485,000.00) inclusive of tax, (50%) of which shall be payable upon the signing of
the contract and the remaining balance to be payable September 11, 2016.

IN WITNESS WHEREOF, the parties have hereunto affixed their respective signatures
at the time and place first mentioned in this agreement.

ACKNOWLEDGMENT

BEFORE ME, Notary Public personally appeared the parties, each exhibiting to me
their respective Government Issued Identification Card known to me and to me known to be
the same persons who executed the foregoing instrument and that they acknowledged to me
that the same is their free act and voluntary deed.

This instrument consists of two (2) pages including this page where the
acknowledgement is written and signed by the parties.
