AFFIDAVIT OF CORRECTION
I, JOANNA T. ESACAÑO, of legal age, married, Filipino citizen and with
residence and postal address at Barangay Caramutan, La Paz, Tarlac, hereby depose
and state – THAT:

1. I am the registered owner of a parcel of land covered by Transfer Certificate of

Title (TCT) No. 043-2018003339 of the Registry of Deeds of the Province of
Tarlac, a copy of which is hereto-attached as ANNEX ‘‘A”;

2. In the said lot title, my civil status was erroneously entered as ‘‘single” instead of
married which is the correct one;

3. In truth and in fact, I am married to CHENG-LUNG CHANG PAN, a Taiwanese

national, a copy of our Certificate of Marriage is hereto attached as ANNEX ‘‘B”;

4. Therefore, it is just my proper that my civil status appearing in the subject lot title
be rectified and changed to ‘‘married to CHENG-LUNG CHANG PAN”;

5. I am executing this Affidavit to attest to the truth of the foregoing declarations and
to support my request for the correction of my erroneously entered civil status in
TCT No. 043-2018003339 from ‘‘single” to ‘‘married to CHENG-LUNG CHANG
PAN”.
