AFFIDAVIT OF CONSENT AND SUPPORT

That we, DIANNE R. ANGARA and JAN MICHAEL B. ENRIQUEZ, both of legal
age, Filipino citizens and presently residing at Laung Cupang, La Paz, Tarlac, after having
been duly sworn to in accordance with law do hereby depose and state:

1. That we are the parents of minor child FRANCES SUMMER A. ENRIQUEZ who
was born on January 20, 2011;

2. That we are giving our consent for our daughter to travel abroad to be
accompanied by my sister, DESIREE R. ANGARA;

3. That our mother who is sponsoring their trip is financially capable to support their
stay abroad;

4. That I guarantee that they will not violate any existing laws of their country of
destination in Kuwait on May 27, 2018;

5. That I am executing this affidavit in connection with my daughter’s application for

a passport at the Department of Foreign Affairs and/or travel clearance from the
Department of Social Welfare and Development for their travel abroad.
