AFFIDAVIT

I,______________ of legal age, Filipinos, and resident of _____________________,


Bacolod City, after having been duly sworn to in accordance with law, depose and say
that:

1.0 I am the biological sister of Jenelly Taño Gumban Cioco and the
granddaughter of Buenvenida Solen Taño and her granddaughter Jenelly Taño Gumban
Cioco.

2.0 As the elder sister of Jenelly, I personally knew that my parents, Spouses
Amado Wayong and Rosa Taño. entrusted to Jenelly’s care to our grandmother,
Buenvenida Solen Taño.

3.0 Buenvenida Solen Taño treated Jenelly as her own. However, unaware of
the legal consequences, Buenvenida arbitrarily changed the name of Jenelly Taño
Wayong to Jenelly Taño Gumban, to honor Buenvenida’s aunt, Anaklita Gumban, who
took care of her (Buenvenida) when she was young.

4.0 Thus, upon the instruction of our grandmother, Jenelly used the name
Jenelly Taño Gumban instead of her real name Jenelly Taño Wayong.

5.0 Consequently, all her school records, from elementary to college, and her
other public records erroneously identified her as Jenelly Taño Gumban.

6.0 It was only after she graduated from college, when she requested for a
copy of her certificate of live birth on security paper with the National Statistics Office
that she found out that her real name as recorded therein is Jenelly Taño Wayong.

7.0 It was only after such discovery that Jenelly started using her real name
Jenelly Taño Wayong in lieu of the name Jenelly Taño Gumban which was arbitrarily
given to her by her grandmother.

8.0 I therefore have personal knowledge of the fact that Jenelly Taño Gumban
and Jenelly Taño Wayong refer to one and the same person. Her correct name is
Jenelly Taño Wayong as appearing in her certificate of live birth on security paper
issued by the National Statistics Office.

9.0 I am executing this affidavit to attest to truthfulness of the foregoing facts and
to support the request of Jenelly Taño Wayong Cioco with the Department of Education
and the Commission on Higher Education to change the name in her diplomas and
school records from Jenelly Taño Gumban to Jenelly Taño Wayong, the latter being her
correct name as supported by her certificate of live birth on security paper issued by the
National Statistics Office.
