AFFIDAVIT OF BUSINESS CLOSURE

I, EMMANUEL GLORIANI, of legal age, Filipino, married and with

residence and postal address at Brgy. Castanos Cerca, Gen.E. Aguinaldo,
Cavite after having been duly sworn in accordance with law, hereby depose
and say:

1. That I am the President of a duly registered business NEW TIGER

EYE HEALTHY FOODS INC. with business address at Halang, Naic,
Cavite;
2. That such business has been properly registered with the government
agencies as mandated by law;
3. That such business has not been operational due to its closure since
December 28, 2017 and I have informed the barangay concerned
about it who issued a certification;
4. That such business has been closed because it was not anymore
profitable, thus its continuation in the business would entail significant
losses; and
5. That I am executing this Affidavit to attest the truth of the foregoing
and for whatever legal purpose this may serve and specifically for
compliance purposes as required by the Bureau of Internal Revenue
and other government agencies for the proper cessation of this
business.
