AFFIDAVIT

I, MARIFE PALANGDAO BAYUDANG, of legal age, Filipino,

married with residence at Cogon, Tineg, Abra after having been duly
sworn in accordance with law, do hereby depose and state:

1. That I was born on April 12, 1982 at Cogon, Tineg, Abra

and a Certificate of Live Birth was registered with the Local Civil
Registrar of Tineg, Abra;

A copy of my birth certificate is hereto attached as ANNEX “A”

2. That when I secured a NSO copy of my Certificate of

Live Birth, I found out that the entry for my Sex/Gender was
erroneously indicated as Male when in truth and in fact I am a
Female;

3. That since birth, my natural and biological gender has

always been FEMALE and that I am identified as a female by my
family, friends, acquaintances and by the community;

A copy of my Marriage Certificate, Community Tax Certificate,

and Medical Certificate are hereto attached as ANNEXES “B, C,
D”, respectively.

4. I am executing this affidavit to attest to the truthfulness

of the foregoing declarations and for the purpose of
solemnization of marriage and for whatever legal purpose this
may serve.
