AFFIDAVIT of COHABITATION
AS REQUIRED ON THE ARTICLE 34 OF THE NEW FAMILY CODE

We X and Y, both of legal age, both single, and residents of P-3 Salvador
Calamba, Misamis Occidental, after being sworn to an oath in accordance with the
law, hereby jointly states:
That we were living together in concept of husband and wife for more than 5
years since February 4, 2013 without the benefit of marriage and begot a child;

That, we desire and strongly decided to marry each other;

That, we have no legal impediment to enter into a contract of marriage;

IN WITNESS WHEREOF, we have hereunto set our hand on the 18th day
