I, DYNAJANE C. SALAYOG, of legal age, Filipino citizen, married, and a

resident of Brgy. Cabuco, Trece Martires City, Cavite, after having been sworn in
accordance with law aver that;

1. I am the child of DANTE V. CADATAL and NANCY CRUCENA;

2. I have used the name DYNAJANE C. CADATAL, in all of my records and

dealings such as government IDs, school records baptismal certificate and
others of similar nature.

3. I got married on June 11, 2006 to Roderick Salayog;

4. After marriage, I changed my name to DYNAJANE C. SALAYOG;

5. Therefore, the following names refer to the one and the same person:

DYNAJANE C. CADATAL
DYNAJANE C. SALAYOG

6. I am executing this affidavit to attest to the truthfulness and honesty of the

foregoing facts and for all intents and purposes it may serve.
