AFFIDAVIT
I, ROSALINDA ALONZO DACOYCOY, 68 years old, married, Filipino
citizen, and a resident of Barangay Libueg, Camiling, Tarlac, Philippines, after
having been duly sworn to in accordance with law, do hereby depose and state
THAT:

1. I was born on October 19, 1949 to Spouses Marciano Alonzo and

Trifonia Lombridas in Quezon City, Manila;

2. On January 11, 1977, I got married to Larry M. Dacoycoy in Quezon

City, Manila; hence, I became known as MRS. ROSALINDA ALONZO
DACOYCOY;

3. When I was single, I took a Memorial Plan at the St. Peter Life Plan,
Inc. (SPLPI) and I was issued a Policy with No. L12503910D (St.
George);

4. I am executing this affidavit to attest to the fact that the names

ROSALINDA LOMBRIDAS ALONZO, as recorded in my Certificate of
Birth including in the said SPLPI Memorial Plan with Policy No.
L12503910D, and MRS. ROSALINDA ALONZO DACOYCOY, as
appearing in my Identification cards (Voter, Senior Citizen and
PhilHealth ID Cards), refers to one and the same persons, and that is
me, Rosalinda Alonzo Dacoycoy;

5. Finally, I am executing this Affidavit to attest to the truth of the

foregoing statements and for all legal intents and purposes it may
serve.
