Student Portfolio Assessment Tracker


The Assessment Tracker document can be used by teachers to
keep track of which portfolio assessments have been administered to
students. Dates can be entered into each cell indicating the date(s)
the assessment was given or the passing score (4/5 or 5/5) to
indicate that the student passed the assessment. This information
can be transferred onto the Student Data Tracking Sheet in the
student portfolio folders.
3rd Grade Student Portfolio Timeline
Please review the Student Portfolio Timeline to ensure that portfolios
arrive to summer sites in a timely manner. All portfolios should be
delivered to summer sites on or before Tuesday, June 12, 2018.

Be on the lookout for ESOL Summer Maintenance Program applications to arrive via school mail
soon! The deadline for faxing applications to the district office is Friday, April 27, 2018. Please
fax all applications as they are received to (904) 390-2800, ATTENTION: ESOL Department. Please
do not wait until the deadline to fax all applications at once. Blank copies of the application will be
uploaded into the Principal’s Toolbox folder; however, applications should only go home to “Active LY”
students as indicated in the FOCUS portal. Blank copies of the application have been provided should
eligible ESOL students enroll at your school during the fourth quarter. Prior to sending applications
home with students, please make a copy of each student’s application and file it in his/her red ESOL
folder. Eligibility (Active LY Status) will be verified by the district ESOL office.
