In fact there is some cross-linguistic contrastive research to suggest that the foreigner is
at a disadvantage. Even where the grammar and vocabulary may be perfectly adequate, it seems
to be the case that a non-native may tend to transfer the discourse patterns of her native
language to English. It has been suggested, for example, that Asian languages such as Chinese,
Japanese and Korean have different patterns of argument to English [3]. Thus one study found
that those Korean academics trained in the United States wrote in an ‘English’ discourse style,
while their colleague who had trained and worked only in Korea, with a paper published in the
same anthology, wrote in a Korean style with no statement of purpose of the article and a very
loose and unstructured pattern from the English point of view [4]. More generally Hinds has put
forward a widely discussed position that Japanese has a different expectation as to the degree of
involvement of the reader compared to English, with Japanese giving more responsibility to the
reader, English to the writer [5]. It might be objected though that this is relevant only to
languages and cultures which differ greatly to English. However, research on German has
shown that German academic writing in the social sciences has a much less linear structure than
English, to the extent that the English translation of a German textbook was criticized as
haphazard or even chaotic by American reviewers, whereas the original had received no
such reviews on the European continent [6]. Academic respectability in English is evidenced
by the appropriate discourse structure but in German by the appropriate level of abstraction
[7]. Similarly, academic Finnish texts have been shown to differ in the way they use
connectors and previews and are much less explicit than English in their drawing of
conclusions. Spanish also has a similar pattern [8]. English, therefore, would seem to be a more
‘writer-responsible’ language than at least some other European languages.

Fasting during the month of Ramadan among patients with chronic


kidney disease

Ramadan is the ninth month in the Muslim lunar calendar this month Muslims perform
activities such as fasting, tarawih, warning the decline in the Qur'an, seek Laylatul Qadr night,
and the emphasis on reading the Koran, pay tithes and ends with the celebration, or a day of
victory. The month of Ramadan is a religious obligation for Muslims. The month of Ramadan
can be referred to as Syahrut Tarbiyah or Education Month. to maintain the health of our bodies
by eating regularly. Or we are taught in order to set the time when the time to eat, when to work,
when to rest and when to worship. The benefits of fasting is to maintain health, including
keeping physically and organ in our body. One of the organs that take care of is the kidney.
Diseases that can be caused by kidney is chronic renal failure. Chronic Kidney Disease (CKD) is
the process of kidney damage with a span of more than three months. CKD can cause simtoma
form of glomerular filtration rate below 60 mL / min / 1.73 m2 or above that amount but is
accompanied by abnormal urinary sediment. The presence of kidney stones can also be
indicative of CKD in patients with congenital abnormalities such as hyperoxaluria and
cystinuria. The symptoms of deteriorating kidney function are not specific, and may include
feeling generally unwell and experiencing a reduced appetite. Chronic kidney disease can also be
identified when it leads to one of the recognized complications, such as cardiovascular disease,
anemia or pericarditis. However fasting in the chronic kidney disease is not recommended by
doctors.

Fasting in the chronic kidney disease is not recommended by doctors. During fasting,
Muslims are expected to avoid drinking and eating from dawn until sunset every day for one
lunar month. Very little is known about the safety of exposing the patient to fast CKD under
climatic conditions during the fasting month. This study shows that changes in renal function
during fasting insignificant or even improved by the end of the month. In the article I get, there
are the results of comparative studies among people with chronic kidney disease who are not
fasting and fasting. Results of the study was in people with chronic renal failure who fasted had
higher levels of keratin compared with chronic kidney disease who are not fasting. Increased
levels of keratin in people with chronic kidney disease showed fall in glomerular filtration rate
and as a result of a decrease in the kidney's ability to excrete waste products. Increasingly, the
higher creatinine levels.

By looking at the effects of fasting on people with chronic kidney disease, doctors
recommend that you should not need to fast as it can cause increasing keratin in the kidneys to
cause death.

Communication with Anxious Clients

Anxious is a fear that is not clear and is not supported by the situation. When a person
feels anxious, someone is uncomfortable, fear may even have a hunch would get in trouble when
he did not understand why the emotion that threatens to happen to him. No object can be
identified as stimulus anxious.
Anxious to have two aspects, aspects of healthy and harmful aspects, which depend on
the level of anxiety, anxiety experienced long, and how well people pass coping with anxiety.
Anxiety can be seen in the range of mild, moderate and severe. Each level causing emotional and
physiological changes in individuals. Anxiety is different from fear, fear is the characteristics of
the object or source-specific and can be identified and can be explained by the individual.

The nurse is the person who is closest to the client so that we as a nurse should be able to
cope with clients who experience anxiety. What we can do is to communicate with clients. The
communication is therapeutic communication, in this case the communication made by a nurse at
the time of nursing interventions should be able to provide therapeutic benefits for the healing of
the client. Therefore, a nurse must improve the knowledge and skills applicable therapeutic
communication and patient satisfaction that the needs can be met.

Therapeutic communication is the interpersonal relationship between the nurse and the
client, in this connection the nurse and the client acquire a shared learning experience in order to
improve the emotional experience of the client. From some of the above can be understood that
the therapeutic communication is communication that have therapeutic significance for the client
and performed by nurses to help clients achieve back condition adaptive and positive.

Anxiety can arise due to the negative mindset embedded in the subconscious. Mindset it
is mindset and the mindset was formed from human interaction with the environment. Anxiety
experienced by clients can be many different forms for example, when the client knows a disease
that is diagnosed by a doctor, at the time of going to undergo major surgery, at the time after
surgery. For example, the client will perform major surgery, the client because never before
surgery then the client will be scared and imagining things - bad things that will happen at the
time of surgery, then the client will continue to think badly of things that will happen to him.
Anxiety will also affect the physiological condition of the client.

When we begin to communicate with the anxiety disorder first step we do is to give
confidence to each other between the client and the nurse and maintaining client privacy. Their
sense of security, closeness and openness will be greatly strengthened communication and
friendship. The nurse approached by referring clients to talk heart to heart with emotion.
Dialogue is delivered in a gentle and fun that can distract a client that is not focused on anxiety.
In a communication to the client should use good ethics and correct. For example saluted
and introduced himself, calling the client's name or family name, make eye contact, use the
volume sounds good, use of intonation are good, maintaining the dignity of clients, develop
positive behavior, using the techniques of active listening, do not interrupt or interrupt,
explaining in an easily understandable and concise, do not use language that is difficult to
understand, show attention to cues, approaching, or nonverbal communication others such as
smiles, respect the rights of patients, help, and attention, giving the impression that we are
listening and trying to understand what disclosed patient, answered every question patiently and
attentively.

It aims to provide an opportunity for the client to receive the actual situation, the client
feel comfortable and do not feel the anxiety again. In addition, for easy nurses in influencing the
problems experienced by the client.
