IEP Example

Strengths:
_____, overall, is a well-mannered and behaved student who gets along with his teachers and peers easily.
_____completes all homework and class assignments on time. Throughout the math period, _____is able
to complete 3-4 questions on all assignments/tasks in class by the time of review. He tends to solve these
problems independently, however attempts to persevere through the problems before advocating for
himself. In analyzing _____strengths and weaknesses in math, _____presents himself as a
visual/kinesthetic learner where he learns best from seeing and doing. _____ attention tends to wander at
times, therefore the kinesthetic approach to learning will greatly assist him; this was especially apparent
in the beginning of the year, however not as much presently. _____ also benefits from having the resource
of process cards and graphic organizers to use as an aide, which he often feels comfortable with. In class,
_____ does participate on multiple occasions and demonstrates the behavioral traits of being an eager to
learn student who understands when it is “work time”. _____ has strong literal comprehension skills, but
struggles on his abstract thinking skills and on higher leveled questions. Overall, _____ does well on
strictly computational concepts.

Present Levels:
_____ is presently in a co-teaching grade level math class with 21 other students. At this time, _____ has a
70.3% (C-) thus far in the third marking period, and has completed all of his homework and ALEKS
assignments. _____ presents a high level of self-determination within the math classroom during lessons,
however struggles in his math fluency, understanding of concepts, differentiating the concepts, and retention of
the concepts. _____ does not have a good understanding of the integer rules which impedes upon the processes
in other concepts. The same goes for fraction operations. _____ receives moderate to high levels of assistance
during math instruction and on assessments. _____ often requires more assistance but does not advocate such
for himself until he is completely unsure. He writes most notes and strategies down without being cued, yet
does not utilize provided resources on his own. _____ often asks to use the resources that he feels most
comfortable with and those that he finds benefits him. _____ is weak in his basic math fluency, which
greatly impacts him within the 7th grade concepts. Additionally, _____ has a low foundation and retention
of math vocabulary and the concepts in relation to one another, which creates difficulty when having to
solve the higher leveled questions and word problems. _____ greatly struggles with the fluidity of
concepts and understanding how concepts compliment and build off one another. _____ has performed in
the below average range on most concepts, such as operations with integers and rational numbers, percent’s,
word problems, vocabulary, and equations. On _____ I-Ready, he initially scored a 490 and recently scored a
487. Overall, _____ scored at a 6th grade level; he scored within the 5th grade realm within Algebraic Thinking
and a 6th grade level in Measurement and Data, Geometry, and Number and Operations. _____ receives the
modified versions of tests along with process cards and a calculator. From my observations and data, I believe
_____ does well within the ICR setting but needs to increase his work habits for stronger grades.

Needs:
_____ would greatly benefit from continuously practicing his integer rules and rational numbers rules in
school and at home. These rules are integrated in all higher leveled concepts from now throughout his
whole mathematics life. Additionally, _____ would benefit from receiving intervention and practicing
operations including percent’s; finding tax, tip, markup, commission, discount, and simple interest. _____
would benefit from relating such to real world examples and experiences in order to promote retention.
_____ would also benefit from using process cards to assist him in solving one, two, and multi-step
equations; the process card will assist in the retention of the steps and processes depending on the type of
equation. Lastly, _____ should practice applying his problem-solving strategies in order to better decode
and break apart word problems. By doing so his likelihood of solving such correctly will increase as he is
able to break it down and comprehend it piece by piece. The same goes for his retention of math
vocabulary; this will guide him in his understanding of word problems and directions.
