This confidential affidavit must be completed for all visa student applicants. Each original bank letter submitted with this affidavit
must contain an amount that meets or exceeds the minimum amount specified on the back of this form, and it must be in English or
have an official English translation attached to the statement. You should have your sponsor obtain two bank statements; one for
CSULB and one for the U.S. consulate. Please print legibly or type.

Private Sponsor information

The business/corporate sponsor must submit proof that the person named below has the authorization to use company funds for

sufficient funds are available to cover the student's educational and living expenses (US $19,420.00 for undergraduates and
