Sample Diagnostic Items – Math

We know it can be helpful for teachers and administrators to see examples of Diagnostic Assessment items, so we have
provided you with this document. Below, you will find a sample item for certain domains within certain grade levels. The
i-Ready Diagnostic draws from a bank of thousands of items when creating each unique test, so this is just a selection of
the types of items students will see. Please keep in mind that these are actual test items, so these should not be shared
with students.
A note on Student User Experience: Depending on a student’s reading ability, the questions and answers will initially be
read out loud, and can be repeated if a student hovers over the speaker icon. You will notice for levels 6–8, there is no
Read Aloud function. After students select an answer, the answer turns orange and they click “Done” to move on to the
next item.

Sample Standard: Represent addition and
subtraction with objects, fingers, mental
images, drawings, sounds (e.g., claps),
acting out situations, verbal explanations,
expressions, or equations.

Measurement & Data


Sample Standard: Draw a picture graph and
bar graph (with single unit-scale) to represent a
data set with up to four categories. Solve simple
put-together, take-apart, and compare problems
using information presented in a bar graph.

Sample Standard: Use place value
understanding to round whole numbers to the
nearest 10 or 100.

Geometry
Sample Standard: Partition shapes into parts
with equal areas. Express the area of each part
as a unit fraction of the whole. For example,
partition a shape into 4 parts with equal area, and
describe the area of each part as 1/4 of the area of
the shape.

Sample Standard: Solve linear equations and
inequalities in one variable, including equations
with coefficients represented by letters.

Algebra & Algebraic Thinking


Sample Standard: Explain why the
x-coordinates of the points where the graphs
of the equations y = f(x) and y = g(x) intersect
are the solutions of the equation f(x) = g(x); find
the solutions approximately.

Geometry
Sample Standard: Prove theorems about
lines and angles. Theorems include: vertical
angles are congruent; when a transversal
crosses parallel lines, alternate interior angles
are congruent and corresponding angles are
congruent; points on a perpendicular bisector of
a line segment are exactly those equidistant from
the segment’s endpoints.
