RE: Requesting medical assistance for Chronic Kidney Disease “Stage 5.”

Dear Honorable Russel M. Abonado ,

I am Francis M. Malasador, 30 years of age, youngest of three siblings and a resident of Purok 4,
Magsaysay Avenue, Poblacion A, Mlang Cotabato.

I was referred by a friend to your good office with regard to our current situation which had been a
dilemma to our family. Few months ago my brother VINCENT B. MALASADOR was diagnosed with
Chronic Kidney Disease that led his decision to rest for a while as he was with no choice but forced to
stop working. He is the bread winner of the family, supporting the basic needs of our mother who is also
on her late years. Our father died not long ago so my brother took the responsibility as the father of the
household since.

It was without any clear warning that Vincent’s Chronic Kidney Disease reached stage 5 and it has been a
while that he was undergoing Hemodialysis treatment, a very expensive procedure. His life savings has
been used up and we are in a great financial turmoil at the present. My sister and I does not have any
stable jobs to sustain his medication and our income is only limited to our basic needs for everyday
living.

We wanted to extend his life especially for our mother who is so dear for her. Two months ago a family
member died and the family is still on the stage of recovery both financially and emotionally.
Attached in this letter are the documents (receipts, medical bills, medical findings) provided by the
hospital as proof.
