1. My child is a Grade 6 pupil of a public/private school that is duly recognized by the Department of Education.
2. We (parents/guardians and applicant) are residents of Taguig City, and have our barangay certificate/ billing statement
3. My child belongs to the upper 10% or top 10 of the batch (SY 2017-2018), and does not have a grade below 85% in major
subjects (Math, English and Science) and below 83% in minor subjects; does not belong to the upper 10% or top 10 of the
batch (SY 2017-2018), but does not have a grade below 85% in major subjects (Math, English and Science) and below 83% in
minor subjects (supported with certification from the principal).
4. We fully understand and accept that the grades obtained in the succeding grading periods (3rd and 4th) lower than the
grade requirements will be a ground for disqualification.

5. We have the certified true copy of Report Card (Form 138) with complete grades for the first to the second quarter of
the current school year duly signed by the principal.
6. We have the certificate of Good Moral Character from the school principal and/or guidance counselor.
7. We have the photocopy of the Registered Birth Certificate (NSO).
