1. SCOPE

This Code of Practice sets down the principles to be applied to secure safety
by the application of special procedures or method statements, this facility
is designed to permit work to be done in exceptional circumstances where
the normal application of the Company Safety Rules is inappropriate.

2. DEFINITIONS

Terms printed in bold type are as defined the Company Safety Rules.

3. PLANT AND APPARATUS IDENTIFICATION

Plant and Apparatus must be readily identifiable or have fixed to it a


means of identification which will remain effective.

4. DANGERS

Danger to personnel and Plant/Apparatus may occur from making


adjustments to, or working on, Plant/ Apparatus in an uncontrolled or
unsafe manner where normal isolation and safety documentation
procedures have not been applied.

5. PHILOSOPHY FOR UNDERTAKING WORK

The Company Safety Rules state in General Provisions 4 (GP4) -


Exceptional Circumstances that:

"Work on Plant and Apparatus which is part of the System and to which
these Rules cannot be applied, or for special reasons should not be applied,
should be carried out in accordance with an Approved written procedure".

When used in this Code of Practice, the term "work" may include testing.

6. LIMITS OF WORK

A Safety Controller shall determine the limits of work to be undertaken


under a GP4 procedure and ensure that these are clearly defined in the
procedure or method statement.

The use of GP4 Procedure shall only be allowed if the work is planned to be
completed within the Designated Persons working day or shift. If the work
overruns, and the Plant/Apparatus has to remain isolated, then the
Designated Person shall inform the operator of the situation.

A Safety Controller shall isolate the Plant/Apparatus and issue a Safety


Document to the Designated Person in charge of the work.

7. PERSONS

The experience and technical ability of Persons who may apply GP4
procedures shall be that of a Designated Person.

Furthermore, Designated Persons are the only persons allowed to work


on Plant/Apparatus isolated under a GP4 Procedure.

8. ISSUE OF GP4 PROCEDURES

A written method of isolating and working shall be produced by the


Designated Person who wishes to carry out the work.

The method and isolation scheme and other safety precautions will be
vetted by a Safety Controller who will append his signature and date to
the Procedure.

The Procedure will be Approved by a Senior Manager nominated by the


Site Manager. This officer Shall sign and date the Procedure.

This Procedure will also be designated as either a repeat or unique status.


The unique status procedure shall only be applied for work on the date
and

time shown. If it is considered that the Procedure is a repeatable


procedure, it shall be added to a suite of site GP4 Procedures for future
use.

The Designated Person applying the GP4 procedure shall:

• Have in his possession the GP4 Procedure

• Obtain permission from the operator to release the Plant/Apparatus


for work. The operator should record the release.

• The Plant/Apparatus shall be where practicable, be immobilised and


locked by unique locks and tagged.

• The contents of the Plant/Apparatus shall be adjusted to a level


which avoids Danger and where Drains could give rise to Danger
they shall be tagged and where practicable locked in the
appropriate position.

• Where Danger could arise from pressurisation, the Plant/Apparatus


shall be vented to atmosphere pressure and where vents could give
rise to Danger, they shall be tagged and where practicable locked
in the appropriate position.

• Where Danger could arise from the released of stored energy, action
shall be taken as per the Approved Procedure to dissipate this
energy safely.

The work may then be carried out as per the Approved Procedure with
the keys to all the points of isolation, correctly identified and in the
Personnel Possession of the Designated Person in charge of the work
