Dear Madam,

This is Mardheeya S. Nuruddin, an incoming freshman of UP College of Law. As part of the

registration, we were given instructions to submit documentary requirements. Because I am
situated in Zamboanga City, I asked a friend to submit my requirements which your office
accepted.

However, I would like to request for an extension with regards to the submission of medical
certificate. I was able to have my medical check up outside the university but the University
Health Service is requesting my personal appearance to submit those. Unfortunately, I am not in
the position to go to Manila and personally submit them. This is because of the current situation
of Zamboanga City as we are affected by Martial Law. Morever, travelling to Manila is difficult
considering the distance and that we are fasting in this month of Ramadhan.

I am hoping for you kind consideration.
