AFFIDAVIT OF DISCREPANCY
I, ANGELA COLAGO LAGUARDIA, Filipino, of legal age,
single and a resident of Cabugao, New Lucena, Iloilo, after having
been sworn to in accordance with law, depose and say:

That in the records of the Municipal Civil Registrar of New

Lucena, Iloilo, October 29, 1968 appears to be as my date of birth;

That my true and correct date of birth is October 29, 1972 as

shown in my Certificate of Live Birth issued by the National Statistics
Office, a certified true copy of which is marked and attached hereto
as Annex “A”;

That I am executing this Affidavit in order to attest to the truth

of the facts herein stated.
