APPLICATION FOR PROBATION

Comes now, Juan Dela Cruz, through the Public Attorney's Office
Masbate City District Office, by the undersigned Public Attorneys, unto
this Honorable Court, most respectfully states that:

1. In a Decision promulgated by this Honorable Court on February

12, 2018, Juan Dela Cruz was held guilty of the crime of Viol. of
Sec. 3, par. (a) of R.A. 9287 and was sentenced to suffer the
penalty of imprisonment of forty-five (45) days as minimum to
ninety (90) days as maximum;

2. The accused, through counsels, hereby manifests his desire to avail

of the benefits granted by the Presidential Decree No. 968,
otherwise known as the “Probation Law of 1976, as amended”;

3. That he is not among those disqualified offenders as enumerated in

Sec. 9 of P.D. 968 as amended by R.A. 10707;

4. That he undertakes to comply with such terms and conditions as

this Honorable Court may impose if his application for probation
will be granted.

WHEREFORE, in light of the foregoing, it is most respectfully

prayed of this Honorable Court that the execution of sentence of accused
Juan Dela Cruz be suspended and that he be placed on probation.

Other reliefs just and equitable are likewise prayed for.
