AFFIDAVIT
I, PCI Joseph R. Baltazar, of legal age designated as Chief Provincial Tactical
Operation and Intelligence Center; Ilocos Norte Police Provincial Office after having been
duly sworn to in accordance with law, do hereby depose and state the following:
This has in reference to memo received by the undersigned from Deputy Provincial
Director for Operations (DPDO) directing me to explain in writing within 24 hours upon
receipt hereof.
That on the night of 13 February 2018, I have already printed my explanation
regarding the memo from DPDO dated 13 February 2018;
That on in the morning of 14 February 2018, I attended the Zumba activity of the
INPPO and had to supervise the dismantling and safekeeping of the base radios and
other equipment used in the venue of the said activity which ended at about past eight in
the morning;
That I had to go home to change my athletic attire to goa type uniform;
That when I went back to the office, I saw and realized that the submitted
explanations of my subordinates were duly notarized;
That I took the initiative to have my explanation be notarized as well for the sake
of uniformity;
That I proceed to the law firm of a notary public, in the person of Atty. Erme S.
Labayog, located near the capitol;
That I was stuck on heavy traffic on my way to the camp due to the funeral parade
at Rizal Street and finally made it to camp at about 11;20 am;
That I was then ready to hand our explanation to the DPDO however, he is having
lunch at the Admin, and therefore I have to hold on to our explanation as a way of respect
to the DPDO;
That I instructed PO1 Adlay Respicio to contact PO1 Rang instead through PTOIC
cellphone issue, however he cannot be reached;
That I instructed PO1 Agoy, duty at the lobby, to submit such explanation to PO1
Rabang when he see him;
That I pray that these explanation will merit your kind consideration.
