AFFIDAVIT of WITNESS

Affiant, ______________________, Filipino, of legal age, and a

resident of Barangay De Ocampo, Trece Martires City, Province of
Cavite, after having been duly sworn to in accordance with law,
hereby depose and state THAT:

1. Affiant as a resident of Barangay De Ocampo and a

concerned citizen of Trece Martires City, that highly regards the
preservation of the environment and ecological system as well as that
of the safety and health of the people, is hereby corroborating and
confirming the allegations of Mario C. De Villa, in the latter’s
Complaint-Affidavit, that the solid wastes in the aforesaid barangay
have not been collected for several months and there was no proper
implementation of solid waste management as stated and verified in
the CENRO Ocular Inspection Report dated 30 January 2018.

2. Affiant is executing this Affidavit for the purposes of attesting

to the truth of the foregoing statements, to inform the proper
authorities of the above facts, and to support the prayer of
Complainant Mario C. De Villa to hold criminally and administratively
liable Barangay Chairperson ROMEO L. MONTEHERMOSO Jr. of
Barangay De Ocampo, Trece Martires City, for violation of the
provisions of RA 9003, its Implementing Rules and Regulations and
the pertinent provisions of Trece Martires City Ordinance No. 2017-
267 or for such other crimes or offenses this Honorable Office may
rule appropriate under the given circumstances.

AFFIANT FURTHER SAYETH NAUGHT
