Addendum:

8. Creditor’s claim is for

a. The debtor failed to pay creditor the financial incentive in accordance with Law No.
70, July 2, 2010, “Incentive, Retirement and Retraining Program Law” Sect. 4, sect. 6
Liquidation of vacation leave, sick leave, retirement pay off and bonuses.
Compensatory time/overtime in accordance FLSA and others.
b. The claimed amount is also for unadjusted pension that continue to accrue every
month.
c. The creditor is unable to specifically its claimed amount.(The debtor refused to
certificate the amount see Exh. 1)
d. The creditor reserves the right to supplement and/or amend the specifics of the claim
and the amount claimed.
e. The creditor reserves the right to provide additional supporting documents upon
request.
