AFFIDAVIT

I, HANEDA EBARONDO YAMADA, of legal age, after having been duly sworn
to in accordance with law, do hereby depose and state:

1. That I am the biological mother of LHEONEY BIEN EBARONDO and

MARK JOSHUA EBARONDO, who was born on March 19, 1997 and January 2,
1999, respectively;

2. That my marriage with their biological father, Leo Angelo Jaucian Tosoc
was already declared null and void as promulgated by the Regional Trial Court of
Pasay City, Branch 113 dated December 27, 2013, and have already attained
finality;

3. That due to erroneous recording and entry of the name and information
in the registration of birth of LHEONEY BIEN EBARONDO and MARK JOSHUA
EBARONDO, the same resulted into discrepancies in the names they used in their
public and private documents;

4. That the name LHEONEY BIEN EBARONDO and LEAH JOHANN

EBARONDO TOSOC refers to ONE AND THE SAME PERSON;

5. That the name MARK JOSHUA EBARONDO and MARK JOSHUA

EBARONDO TOSOC refers to ONE AND THE SAME PERSON;

6. That I, HANEDA EBARONDO YAMADA, LHEONEY BIEN EBARONDO

and MARK JOSHUA EBARONDO are all connected to each other as family;

7. That I have executed this Affidavit to attest to the truthfulness and

veracity of all our foregoing statements and for whatever legal intents and
legitimate purpose it may serve, especially for all authorities concerned.
